Changing primary care providers, whether because of a move or insurance change, can be stressful. Did you find the right provider? Will they take good care of you? All of these questions are important, and there are a few things to keep in mind when starting as a new patient at Value Care Health Clinic.

  1. Medical Records – If you have medical records from previous clinics, you will want to start that transfer process as soon as possible. Our clinic cannot start renewing prescriptions or giving referrals until we have that information, so as soon as you can start giving us that information, the better.
  2. Health Insurance – When you get your first appointment, you will need your current insurance information card available, both for you and for all patients that will be getting care at our clinic. We will scan this and keep a copy in your file. For children under 18, they will also have their own insurance card with Medicaid and their own Medicaid number.
  3. Photo ID – Bring this with you to your first appointment because we make a copy of this for your file. Please bring one for every patient that will be seen at our clinic. If you have children under 18, we can keep a copy of yours in their file if they don’t have one.
